Prašome užpildyti šią registracijos formą prieš atvykstant į renginį Nepadorūs skaitymai | Moterys ir vyrai lietuvių kultūroje, vyksiantį gruodžio 8 d. 19 val. Kauno menininkų namuose. Primename, kad privaloma dėvėti medicininę nosį ir burną uždengiančią kaukę. Kaukės renginyje nebus dalijamos, tad prašome atvykti jomis pasirūpinus iš anksto. Dalyviai be registracijos nebus įleidžiami į renginį. Renginys vyks lietuvių kalba.
Jūsų asmeniniai duomenys bus tvarkomi tik COVID-19 ligos (koronaviruso infekcijos) profilaktikos ir kontrolės priemonių įgyvendinimo tikslais ir saugomi 21 dieną nuo įvykusio reginio dienos, o po to iš karto sunaikinti. Jūsų pateikti duomenys gali būti perduoti tik Nacionaliniam visuomenės sveikatos centrui pateikus prašymą, epidemiologinės diagnostikos tikslais.

Due to safety requirements, we ask everyone to register prior to coming to the event. Please fill in the form with your details to register for the event Obscene Readings | Women and Men in Lithuanian Culture on 8th of December at 7 p.m. at Kaunas Artists' House. We remind you that it is obligatory to wear a medical mask that covers nose and mouth. There will be no masks available at the event so we kindly ask you to arrive with a mask already. Not registered people will not be admitted to the tour. The event will be in Lithuanian.
Your personal information will be used only in COVID-19 disease (corona virus infection) prophylactic and control purposes and will be kept safe for 21 days after the day and destroyed afterwards. Your personal information may only be transferred to National Health Centre in case of request for diagnostics.
