Dėl jūsų pačių saugumo, norint dalyvauti renginyje privaloma registracija. Prašome užpildyti šią registracijos formą prieš atvykstant į parodos „Belaukiant krypties“ atidarymą, vyksiantį rugsėjo 9 d. 18 val. VDU menų galerijoje „101“, Muitinės g. 7. Primename, kad būtina dėvėti medicininę nosį ir burną uždengiančią kaukę. Kaukės renginyje nebus dalijamos, tad prašome atvykti jomis pasirūpinus iš anksto. Žiūrovai be registracijos ir/ar be kaukės nebus įleidžiami į renginį.
Jūsų asmeniniai duomenys bus tvarkomi tik COVID-19 ligos (koronaviruso infekcijos) profilaktikos ir kontrolės priemonių įgyvendinimo tikslais ir saugomi 21 dieną nuo įvykusio reginio dienos, o po to iš karto sunaikinti. Jūsų pateikti duomenys gali būti perduoti tik Nacionaliniam visuomenės sveikatos centrui pateikus prašymą, epidemiologinės diagnostikos tikslais.

Due to safety requirements, we ask everyone in audience to register prior to coming to the event. Please fill in the form with your details to register for the opening of exhibition "Waiting for Directions" on the 9th of September 6 pm at Vytautas Magnus University Arts Gallery "101", Muitinės g. 7. We remind you that, it is a new government requirement to wear a medical mask that covers nose and mouth. There will be no masks available at the event so we kindly ask you to arrive with a mask already. Not registered or/and people without a mask will not be admitted to the event.
Your personal information will be used only in COVID-19 disease (corona virus infection) profilactic and control purposes and will be kept safe for 21 days after the day and destroyed afterwards. Your personal information may only be transferred to National Health Centre in case of request for diagnostics.
