ISO/IEC 18004 rule: Minimum QR Code size = scanning distance ÷ 10. A QR Code scanned from 30 cm needs to be at least 3 cm × 3 cm. This assumes a modern smartphone camera. Older devices or cameras with lower resolution may require larger sizes.
